Our Haude clinic assistant is Jessika Bonnet, Medical Asst. She is bilingual and can assist in translating Spanish. You can reach Jessika at 832-484-5609.
***Please bring or email annual health provider paperwork, physician orders, immunization records, etc. to school beginning 2 weeks before school starts each year. If I am not available, the front office staff will place paperwork in my mailbox. (faxing is not recommended during the summer because the machines may not be working.) I will not be answering emails until I return each August.
Medication authorization forms can be brought to the clinic or completed in the clinic when you bring the medication starting the week before school starts. Please call ahead to make sure the nurse or clinic asst will be available. Thank You!
